CAPE TOWN - Correctional Services Commissioner, Samuel Thobakgale, has appointed two officials to investigate security breaches at Pollsmoor Prison in Cape Town.
Two inmates were erroneously released.

An administrative oversight temporarily gave John Henry Mpelo his freedom, while Inganathi Daba got out using another detainee's name.
Both men were re-arrested within two weeks.
